My Logbook is your personal record of 
achievement in the Swim and Survive program.
Skills achieved will be marked off by the Swim 
Teacher. Once all the listed skills for the award 
level have been successfully completed, the 
Swim Teacher will sign and date the logbook 
for that award level and you will be ready 
to progress to the next level in the Swim and 
Survive pathway.
